About Us

Join the London Borough of Barnet as a Lead Practitioner in our Social Care Direct Team. This unique, non-caseholding role offers the chance to lead and manage the team responsible for handling and directing referrals received via email. With a hybrid working model, enjoy the flexibility of working remotely for two days a week.

Key Responsibilities
  • Assess and prioritize incoming emails to Social Care Direct, providing notes and guidance for team action.
  • Distribute emails equitably among team members to ensure efficient workload management.
  • Offer support and advice to team members on email assignments to facilitate informed decision-making.
  • Identify and direct appropriate processes and pathways, including reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge.
  • Screen emails meticulously to determine their suitability for referral to Social Care Direct, ensuring compliance with protocols.
Requirements
  • Professional qualifications in Social Work, registered with Social Work England.
  • Experience in social care practice, especially in triaging and directing referrals.
  • Strong organizational skills for managing email correspondence and workload allocation.
  • Excellent communication skills to provide clear guidance and support to the team.
  • Knowledge of adult social care processes and pathways, such as reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge.
