Key Responsibilities:
Visiting patients in their homes to provide assessments and interventions
Conducting blood pressure checks
Assessing, recommending, and providing necessary equipmentSupporting rehabilitation and reablement plans
Requirements:
Qualified Occupational Therapist with relevant experienceFull UK driving licence and access to a vehicleStrong communication and assessment skillsPassion for supporting individuals in regaining independence
