About the Role:Service Care Solutions is recruiting a Band 5 Community Nurse to join the team in Kidderminster NHS on an agency contract. This role offers an excellent opportunity for a nurse to provide high-quality care within a community setting, supporting patients in their homes or other community environments. Key Responsibilities:

  • Deliver high-quality nursing care to patients in a community setting.
  • Administer medication and treatments in line with EPMA protocols and training.
  • Provide assessments, care planning, and regular evaluations to meet patients' individual needs.
  • Work collaboratively with multidisciplinary teams, including GPs, physiotherapists, and social workers, to ensure seamless patient care.
  • Support patients and their families with education, advice, and emotional support regarding their care plans.
  • Manage a caseload of patients, prioritizing and organizing work effectively.
  • Maintain accurate and timely records of all patient care, adhering to professional and legal standards.
  • Participate in team meetings and contribute to service development initiatives.

Requirements:

  • Registered Nurse (Band 5) with a valid NMC PIN.
  • Previous experience in community nursing is highly desirable.
  • EPMA training is essential.
  • Strong communication and interpersonal skills.
  • Ability to work independently as well as part of a team.
  • Flexibility to work both day and night shifts, as required.

Working Hours:

  • 3-4 shifts per week, including a mix of days and nights.
