As a Community Nurse Case Manager, your responsibilities will include:
- Conducting initial assessments in the community
- Providing clinical oversight to your team
- Collaborating with the wider multidisciplinary team (MDT)
- Attending meetings and making necessary referrals
- Managing a team of non-clinical case managers
The Community Nurse Case Manager role comes with a comprehensive package:
- Annual salary of £48,000

- Monday to Friday, 9am-5pm with some flexibility
- Two days home-based, three days community visits

The ideal candidate for the Community Nurse Case Manager role will have:
- RGN qualification with excellent clinical skills in complex care
- Experience with peg feeds, ventilators, and tracheostomy management
- A valid driving licence, as the role involves remote and community-based work
