About the Senior CHC Nurse Assessor Role

As a Senior CHC Nurse Assessor, you will be a key member of our Continuing Care and Learning Disability Team. This role offers a chance to utilise your nursing expertise in a dynamic and impactful way, ensuring that children and young people receive the care they need.

Key Responsibilities of the Senior CHC Nurse Assessor:

  • Lead multi-disciplinary assessments and reviews for children and young people's continuing care
  • Coordinate transition planning for young people aged 16-25 years as a Senior CHC Nurse Assessor
  • Contribute your expertise to joint commissioning for special educational needs and disability (SEND)
  • Develop and manage personal health budgets in your capacity as a Senior CHC Nurse Assessor
  • Ensure compliance with national frameworks, NMC Code of Professional Conduct, and organisational policies
  • Build and maintain constructive relationships with internal and external stakeholders
  • Participate in safeguarding processes and SEND pathways

What We're Looking for in a Senior CHC Nurse Assessor:

  • Registered Nursing Qualification with current NMC registration
  • Degree level education in a relevant subject or equivalent experience
  • Extensive knowledge of the Special Educational Needs and Disability (SEND) code of practice
  • Experience in child development and complex care needs for ages 0-25 years
  • Understanding of safeguarding processes and information governance

Desired Qualifications for the Senior CHC Nurse Assessor Role:

  • Paediatric qualification (e.g., RSCN or RN Child)
  • SCPHN qualification
  • First-line management experience or qualification

Skills and Competencies Essential for a Senior CHC Nurse Assessor:
