Job description:

Assessment and treatment/discharge planning for patients admitted to older adult in-patients’ sub-acute wards.
Potential for being peripatetic across MHH, WHH, PTC and AMH.
Experience of RIO desirable.
Must have experience of Discharge to Assess process.
Car Driver preferable for Access Visits.
