Responsibilities
- Assess needs -Evaluate patients' needs and goals
- Create care plans -Develop proactive care plans for patients
- Monitor care -Visit patients to ensure that the care they are receiving is satisfactory
- Adjust care plans -Revise care plans when there is a change to a clients health
- Conduct reviews - To maintain high standards in terms of service delivery
- Help with transitions -Assist patients with transitions between different types of care
- Support patients -Encourage clients to become more involved in their own care and health
Skills and Qualifications
- Strong communication skills
- A positive attitude
- Proficient in the use of Microsoft
- Good organisational and prioritisation skills
- Ability to work well under pressure
- Attention to detail
- Ability to build relationships
- Relevant experience within the healthcare sector (preferred)
- Must hold a full UK driving license and have access to your own vehicle
