Responsibilities

  • Assess needs -Evaluate patients' needs and goals
  • Create care plans -Develop proactive care plans for patients
  • Monitor care -Visit patients to ensure that the care they are receiving is satisfactory
  • Adjust care plans -Revise care plans when there is a change to a clients health
  • Conduct reviews - To maintain high standards in terms of service delivery
  • Help with transitions -Assist patients with transitions between different types of care
  • Support patients -Encourage clients to become more involved in their own care and health
