Job title: Clinical Screener

Rates: 32 per hour
To be considered for this role you need a minimum of 6 months NHS experience within the last 2 years.
Job Summary RN- Clinical Screener: the post holder will work in partnership with colleagues in the acute sector, social services, continuing care and other community health services to ensure that appropriate patients from the acute setting are assessed and transferred safely to appropriate community based services in a timely and seamless manner. For appropriate pathways there is an aim to support transfer from hospital to community within 2 hours or on the same day. To be responsible for ensuring smooth daily operations at the acute hospital bases and to act as a key point of contact and subject matter expert for community services to support with complex discharge queries. To lead daily huddles as required
The role will be commencing on the ASAP and will be ongoing.
To be considered for this post applicants will need:
* Full NMC registration
* Enhanced DBS
