Winlight Recruitment are currently seeking an experienced Social Worker to join the Hospital Discharge Team at Pilgrim Hospital in Boston, Lincolnshire. This is a fantastic opportunity to be part of a dedicated team supporting patients through the discharge process, ensuring they receive appropriate care and support once leaving the hospital.
Key Responsibilities:
- Support patients in the discharge process, ensuring a smooth transition from hospital to home or alternative care settings.
- Complete comprehensive assessments for patients, identifying their needs and developing tailored care plans.
- Collaborate with healthcare professionals, families, and community resources to arrange appropriate post-discharge care.
- Liaise with external agencies to ensure the necessary support services are in place for patients.
- Maintain accurate records and documentation in compliance with hospital policies and procedures.
- Work closely with the multidisciplinary team to provide holistic and person-centred care to patients.
Requirements:
- Qualified Social Worker with SWEregistration.
- Proven experience in hospital discharge and working within a healthcare setting.
- Strong communication skills and the ability to work effectively as part of a team.
- Ability to work autonomously and manage your caseload effectively.
- A commitment to providing high-quality, person-centred care.
- Full-time availability to work on-site 5 days per week (37 hours).
- Driving License and access to a vehicle (preferred but not essential).
