Your new companyWe are seeking experienced Social Workers with a background in Acute Hospital settings to join a dedicated Adult Social Work team in Bournemouth on a full-time, locum basis. Your new roleYou will have a proven track record of working in fast-paced hospital environments and be committed to providing high-quality care to patients.

  • Provide on-site support at the hospital 50% of the time.
  • Travel to review clients who have been discharged from the hospital.
  • Collaborate with healthcare professionals to ensure comprehensive patient care.
  • Conduct assessments and develop care plans for patients.
  • Work as part of a multidisciplinary team to safely discharge adults 18 years and older.
  • Follow the Discharge to Assess (D2A) model, ensuring needs are assessed post-discharge.
  • Supporting individuals with complex needs, participating in ward discharge planning meetings, Multi-agency Risk Management Meetings (MARM's), and Best Interest Meetings.
  • Support front of house services (A&E and assessment units) to prevent unnecessary hospital admissions

What you'll need to succeed

  • Previous experience working in an Acute Hospital environment is essential.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to travel as required.
  • Social Work Degree or equivalent qualification.
  • Social Work England Registration.
