Our services focus on promoting independence and helping young people prepare for their transition into independent accommodation.

Role Responsibilities

As a Project Worker, you will:

  • Manage a caseload of 10-12 young people, conducting key working sessions to create SMART Support Plans.
  • Complete thorough needs assessments, risk assessments, and management plans.
  • Organise, attend, and contribute to multi-agency meetings.
  • Assess referrals to ensure suitability and criteria are met.
  • Assist young people in finding appropriate move-on accommodation, including private rental, shared housing, or self-contained units.
  • Facilitate mediation with families or other professionals to help young people return home when appropriate.
  • Work collaboratively with internal teams and external agencies to address housing, mental health, and other support needs.
  • Help young people maximise benefit entitlements and support rent/service charge collection.
  • Support clients ready to enter work, education, or training by connecting them to relevant opportunities.
