Service Care Solutions are recruiting for an Advanced Nurse Practitioner to work within an GP Practice, in Bristol, on a permanent basis.Advanced Nurse Practitioner Contract Type: Permanent Pay: Negotiable + £250 Welcome Bonus Hours: Full Time - 37.5 Hours | Mon - Fri

Duties:- Work within the MDT, delivering care within their scope of practice to the entitled patient population.- You will be working autonomously, will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.- As part of this role you will develop, implement and embed health promotion and wellbeing programmes and develop, implement and evaluate individual treatment plans for acute and non-acute conditions, for both Adults and Paeds.- You will support the clinical team in the development of individual treatment plans for chronic disease patients and Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.- Providing routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF is a must and you will be able to prescribe medication in accordance with PGD and within own scope of practice and review medication for effectiveness, need and national guidance

Essential

  • Experience of working as an advanced nursing practitioner
  • Experience of dealing with vulnerable patients
  • Experience in managing minor illness presentations in children from age 6 months
  • Registration with the NMC
  • Meets the standards for registered ANP working at advanced level
  • Minor illness qualification
