Our client Bromley Council, is looking for an interim programme manager to support adult social care throughout winter to meet and respond to seasonal demands and mitigate the impact on social care provision ensuring statutory responsibilities continue to be met.

The post holder will oversee the mobilisation of winter funding as well as support the system where necessary for example supporting the review and development of operational pathways where additional demand or delays may be identified, or working with the adult social care provider market to meet increase in demand during the period.

SUMMARY OF RESPONSIBILITIES AND DUTIES:

  • Manage LBB winter funding, deploying it to meet presenting needs ensuring increased capacity meets need, is delivered within the financial envelope and achieves maximum impact
  • Develop winter capacity and demand live dashboard to allow system monitoring and management
  • Identify system challenges, lead transformation and change
  • Developing social care pathways to reduce pressures on acute based care
  • Programme manage the roll out ff re-directed acute funding, harnessing innovation and enhancing the current national best practice to gold standard model of care
  • Work with senior leadership appropriately escalating and providing solutions and leading change
  • Developing integrated models of care that can support the system to respond to system pressure
  • Develop and mainstream a long-term model that maintains current performance mobilising the agreed Trusted Assessor infrastructure
  • Provide regular reporting on impact and outcomes as well as financial position maintaining effective programme management documentation to maintain oversight of the activity.
