Job Overview:We are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at MPFT. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.

Key Responsibilities:

  • Assess, plan, and deliver occupational therapy interventions for patients in the community

  • Work with multi-disciplinary teams to support rehabilitation and recovery

  • Develop and implement tailored care plans to meet individual needs

  • Provide advice and education to patients, families, and carers

  • Manage a caseload, ensuring timely and effective interventions

Key Requirements:

  • HCPC registration as an Occupational Therapist

  • Proven experience in community-based occupational therapy

  • Strong communication and interpersonal skills

  • Ability to work independently and as part of a team

  • Experience in managing complex caseloads is desirable
