About the Role

We are seeking a dynamic and enthusiasticGeneral Practitionerto contribute to our clients innovative, multi-disciplinary approach to patient care. As part of the ARRS initiative, you will work closely with a team of healthcare professionals, includingClinical Pharmacists,Paramedics,Physician Associates, andSocial Prescribing Link Workers, to enhance the services we provide to our diverse patient population.

Key Responsibilities

  • Deliver high-quality, patient-centered care in collaboration with the PCN team.
  • Support the management of long-term conditions, medication reviews, and acute presentations.
  • Contribute to clinical innovation and service development.
  • Play a key role in improving access and outcomes for patients within the network.
  • Engage in population health management and proactive patient care planning.
