Social Worker D2A

Belmont Recruitment are currently looking to speak to Registered Social workers for a Role Based In Southern Staffordshrie working as Part of a D2A, Discharge to Assess team.

Belmont recruitment are working with a Local Authority within Southern Staffordshire to fill a D2A role working on a Locum 3 Month + Rolling Contract.

This role is Full time Hours across 09:00-17:00. However for the right candidate the team would consider 4 days PW work.

JOB PURPOSE:

• The post holder will be accountable on a day to day basis to the Team Lead and Advanced Social Work Practitioner who will allocate, monitor and review all work through regular informal and formal supervision.

• The post holder will have a key role to play in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers ensuring legal literacy, Strength and asset based practice, personalisation and social inclusion is embedded and applied effectively throughout their work.

• The role will involve assessing; accessing; and implementing a range of social care services, and providing guidance/advice and information for adults with acute or increasingly complex long term physical health, mental health needs and for older people and carers.

• The postholder will be required to be an integrated and supportive member of a team, planning, actioning, monitoring and evaluating work with the supervision of their line manager and will work to leading a defined area (in accordance with service priorities).

This role is currently Paying the Following Rates:

Days - 25.50

Hours: Full Time
Days: Monday - Friday
Contract: 3 Months (+ Ongoing)
Band: 6
Salary: Upto 25.50 per hour
