Overview: We are seeking a compassionate and dedicated Support Worker to provide high-quality, person-centred support to individuals.The successful candidate will work collaboratively with service users, colleagues, and external professionals to promote independence, well-being, and healthy life choices.

Key Responsibilities:

Service User Support:

  • Promote the rights and responsibilities of service users.

  • Work as an effective team member to ensure the best outcomes for individuals.

  • Provide advice and information to service users and relevant stakeholders.

  • Deliver tailored interventions in line with individual care plans using evidence-based models.

  • Liaise with external professionals such as GPs and Social Services.

  • Maintain accurate records and contribute to service monitoring requirements.

  • Conduct risk assessments and implement effective risk management strategies.

  • Ensure adherence to Health & Safety requirements.

  • Assist in the facilitation of effective care pathways.

  • Support individuals in achieving personal goals in accordance with their care plans.

  • In residential settings, ensure service users take prescribed medication on time, providing assistance when necessary.

  • Facilitate access to community-based services and resources to enhance service users' quality of life.

Quality Assurance:

  • Maintain high-quality standards and meet performance criteria.

  • Participate in data collection and service improvement initiatives.

  • Contribute to the continuous development of the service.

Personal Development:

  • Engage in regular performance management meetings to set and review objectives.

  • Participate in relevant training and professional development opportunities.

Health & Safety & Risk Management:

  • Maintain a safe and clean working environment.
