Job Description:As an Adult Social Worker, you will play a crucial role in reviewing and updating support plans to meet the evolving needs of service users. You will also chair safeguarding meetings when necessary, ensuring that adults at risk are protected and supported effectively.

Responsibilities:

  • Conduct support plan reviews to evaluate their effectiveness and make updates or changes where required.
  • Work collaboratively with service users, families, and other professionals to ensure care plans are person-centred and outcome-focused.
  • Chair safeguarding meetings when required, ensuring that safeguarding procedures are followed and risks are effectively managed.
  • Manage your own diary, allowing flexibility and independence in your workload.
  • Ensure compliance with legal and professional standards while maintaining accurate and up-to-date case records.
  • Engage with a minimal amount of travel to and from the area, focusing your time on impactful work with service users.

Qualifications:

  • Qualified Social Worker registered with Social Work England.
  • Experience in adult social care, particularly in support planning and safeguarding.
  • Strong knowledge of relevant legislation, policies, and procedures.
  • Excellent organisational and time-management skills, with the ability to work independently.
  • Strong communication and interpersonal skills, enabling effective collaboration with service users and professionals.
