- Aware of our responsibilities under the Care Act 2014 in relation to the principles of the Act, prevention, having strengths-based conversations in order to determine eligibility and support planning with service users, families and advocates.
- Aware of the principles of the Mental Capacity Act 2005 with experience of undertaking Capacity and Best Interest Assessments.
- Experience of NHS Continuing Healthcare including undertaking checklists and attending DST meetings.
- A proven ability to manage a caseload effectively.
