Job Title: Band 6 Mental Health Practitioner

Location: Chorley (Community)

Contract: Full-Time, Monday to Friday (9 AM - 5 PM), part-time also considered

About the Role

We are seeking a Band 6 Occupational Therapist specialising in Mental Health to join the CMHT. This role involves assessing, planning, and delivering care to service users, from initial assessment through to discharge, with a strong focus on individualized and holistic care to support recovery and improve quality of life.

Key Responsibilities

Conduct assessments and create personalised care plans that address mental health and functional needs.
Collaborate with service users, carers, and multidisciplinary professionals to implement and evaluate individualised care.
Conduct clinical risk assessments and develop person-centered care plans.
Provide therapeutic interventions and advocate for the needs and rights of service users.
Supervise non-qualified staff and contribute to developing mental health knowledge within the team.
Work closely with community-based services, social services, and healthcare agencies to ensure comprehensive care.
Participate in new patient assessments and help integrate care pathways across services.

About You

Qualifications: Registered Occupational Therapist with experience in mental health.
Skills: Skilled in patient assessment, collaborative care planning, and therapeutic intervention.
Knowledge: Understanding of the Mental Health Act and risk management.
