Job Title: Band 6 Mental Health Practitioner

Location: Chorley (Community)

Contract: Full-Time, Monday to Friday (9 AM - 5 PM), part-time also considered

Start Date: ASAP

About the Role

We are seeking a Band 6 Occupational Therapist specialising in Mental Health to join the CMHT. This role involves assessing, planning, and delivering care to service users, from initial assessment through to discharge, with a strong focus on individualized and holistic care to support recovery and improve quality of life.

Key Responsibilities
  • Conduct assessments and create personalised care plans that address mental health and functional needs.
  • Collaborate with service users, carers, and multidisciplinary professionals to implement and evaluate individualised care.
  • Conduct clinical risk assessments and develop person-centered care plans.
  • Provide therapeutic interventions and advocate for the needs and rights of service users.
  • Supervise non-qualified staff and contribute to developing mental health knowledge within the team.
  • Work closely with community-based services, social services, and healthcare agencies to ensure comprehensive care.
  • Participate in new patient assessments and help integrate care pathways across services.
About You
  • Qualifications: Registered Occupational Therapist with experience in mental health.
  • Skills: Skilled in patient assessment, collaborative care planning, and therapeutic intervention.
  • Knowledge: Understanding of the Mental Health Act and risk management.
