Job Scope:

We are looking to recruit a dedicated and professional Care Coordinator to ensure the efficiency and quality of care services, supporting older adults and individuals with disabilities by offering home care that promotes community engagement. This includes tailored support for individuals with learning difficulties, physical disabilities, dementia, and complex needs. Based in our clients Colchester office, the Care Coordinator will also oversee packages of care in Suffolk and surrounding areas for assessments and, when necessary, provide hands-on care.

As a Care Coordinator, you will develop and manage schedules for service users, ensuring the delivery of reliable and compassionate care. You will supervise and support care staff, conducting regular training to enhance their skills and compliance with health and social care regulations, including CQC standards. You will collaborate with service users and their families to create and review personalized care plans, advocating for their needs and promoting independence and dignity. Additionally, you will maintain accurate records, handle new care package assessments, and adapt to schedule changes, including weekend work on a rota basis, to ensure seamless care delivery. Your empathetic and organized approach will make a meaningful impact on our clients customers' lives and well-being.

Do you have?

  • NVQ Level 3 qualification in Health & Social Care or equivalent experience in healthcare management.
  • At least 1 year of experience in domiciliary care.
  • Valid driving license with access to your own car.
