Key Responsibilities:
- Deliver person-centred care and support, promoting independence and well-being for individuals in your care.
- Assist in creating, reviewing, and updating individual care plans to ensure they reflect the needs of each service user.
- Work collaboratively with external professionals, families, and stakeholders to provide holistic care.
- Participate in meetings, reviews, and care plan updates to support service user development.
- Promote a safe, nurturing, and positive environment for all individuals.
Requirements:
- NVQ Level 2 in Health and Social Care (or equivalent) is desirable.
- Minimum of 1 year of experience in a care or support role.
- Full UK manual driving license is essential.
- Excellent communication and interpersonal skills.
- Knowledge of safeguarding, CQC standards, and relevant regulations.
- A compassionate and empathetic approach, with a genuine commitment to supporting vulnerable individuals.
