Hatched Recruitment is working closely with the local authorities and we are looking for a OT in the North Somerset area. This is to support vacancies in TRI Team and ToC hub pending formal recruitment process.

Occupational Therapist role working within a reablement service for North Somerset Council. MDT team supporting hospital discharges and community reablement. Therapist led service also utilising technology enabled care interventions. Role involves caseload management, assessment and goal setting to determine a reablement plan and short term interventions for up to 6 weeks.

Candidates must have experience working in the community and be familiar with lone working procedures. Access to car or ability to travel around North Somerset required. May also be required to work within UHBW hospital sites as part of transfer of care hub on rota basis dependent on experience.

Familiarity with the care act, social care, manual handling, capacity assessments and assessing care and support needs will be needed to undertake this role.

Knowledge and experience of technology enabled care desired.
