Children’s Family Coordinator, East Riding of YorkshirePay rate, £18.68 per hourVitalis Social Care are recruiting for an experienced Family Coordinator to work directly with and deliver targeted and coordinated interventions to families with children aged 0-19 years (or 25 with SEND) who have complex and interacting needs.Job Role and Responsibilities: - Supporting families against a co-produced, strength-based assessment and family support plan.  You will work with partners to best identify ways to support children and families, including effective information sharing.
