Job Overview:We are seeking a dedicated Band 7 Occupational Therapist to join our SDH&C Home First Service. The role involves attending ward and board rounds, liaising with acute and community services, and conducting follow-up D2A (Discharge to Assess) visits in the community to support patient recovery.

Key Responsibilities:

  • Attend ward and board rounds to provide input on patient care and discharge planning.
  • Liaise with acute and community services to ensure smooth transitions and continuity of care.
  • Conduct follow-up D2A visits in the community to assess and support patients post-discharge.
  • Develop and implement individualized treatment plans based on patient assessments.
  • Collaborate with multidisciplinary teams to enhance patient outcomes and service delivery.

Key Requirements:

  • Degree in Occupational Therapy and current HCPC registration.
  • Significant experience in community and inpatient settings, with a focus on D2A services.
  • Strong communication and organizational skills, with the ability to work effectively in a team.
  • Proven ability to manage complex caseloads and provide patient-centered care.
