  • Develop relationships with service users and accurately assess complex psychosocial needs.
  • Provide complex psychosocial support for those with terminal illnesses and their families.
  • Offer information, support, and signposting related to housing, benefits, finances, and care options.
  • Ensure compliance with safeguarding policies and address the needs of vulnerable adults.
  • Participate in multi-professional meetings and contribute to holistic assessments.
