• Provide high-quality, research-based casework service to adults living in the Bailiwick of Guernsey.
• Conduct assessments using standardised formats and create personalised care plans for individuals.
• Work as part of an integrated adult community care team offering consultation, support, and specialist advice.
• Coordinate services for adults wishing to remain in their own homes or transition into care.
• Develop relationships with service users and accurately assess complex psychosocial needs.
• Provide complex psychosocial support for those with terminal illnesses and their families.
• Offer information, support, and signposting related to housing, benefits, finances, and care options.
• Ensure compliance with safeguarding policies and address the needs of vulnerable adults.
• Participate in multi-professional meetings and contribute to holistic assessments.
