We are seeking an experienced and proactive Education Health and Care Coordinator to join our reputable Further Education College. This role involves coordinating the Education Health and Care Needs assessment process, including consultation and annual review of the EHCP for individual learners. The successful candidate will play a crucial role in developing partnerships with parents, practitioners, and young people and will act as the main contact point for SEND Caseworkers across our partner boroughs.

Day-to-day of the role:
  • Coordinate the Education Health and Care Needs assessment process for individual learners.
  • Consult and conduct annual reviews of the EHCP, ensuring compliance and effectiveness.
  • Develop and maintain partnerships with parents, practitioners, and young people to enhance the support system.
  • Serve as the primary contact for parents, young people, and SEND Caseworkers across partner boroughs.
  • Organise and attend meetings related to EHC planning and transition requirements.
  • Chair Annual Reviews and complete necessary paperwork efficiently and accurately.
  • Monitor and adapt action plans in response to learner needs and compliance requirements.
Required Skills & Qualifications:
  • A good standard of education and thorough knowledge of the Children and Families Act.
  • Understanding of the SEN and Disability Code of Practice 0-25 years and associated regulations.
  • Proven experience in creating, monitoring, and adapting action plans.
  • Experience working with external professionals such as Local Authorities and Schools.
  • Skilled in chairing Annual Reviews and managing related documentation.
  • Excellent communication and interpersonal skills.
  • Proficiency in Microsoft Office, particularly Excel.
