What You’ll Be Doing:

Join our team as a Community Mental Health Nurse (CPN) and play a vital role in supporting adults with mental health challenges. Your responsibilities will include:

? Conducting comprehensive mental health assessments in a community setting.? Developing and implementing personalised care plans in collaboration with a multidisciplinary team.? Providing evidence-based interventions to support patient well-being and recovery.? Offering crisis intervention and risk management strategies to ensure patient safety.? Liaising with families, social workers, and other professionals to deliver holistic care.? Maintaining accurate and up-to-date patient records in line with NHS standards and guidelines.? Offering guidance and psychoeducation to patients and caregivers.

Requirements & Qualifications:

To be successful in this role, you will need:

? A current and valid NMC registration (Registered Mental Health Nurse – RMN).? Previous experience as a Band 6 Community Psychiatric Nurse (CPN).? Strong communication and interpersonal skills.? The ability to work independently and within a multidisciplinary team.? A commitment to delivering high-quality, patient-centred care.? A full UK driving licence and access to a car is essential.

Working Hours & Contract Duration:

This role requires availability from Monday to Friday, 09:00 AM - 5:00 PM. The contract is for an initial 3-month period with a strong possibility of extension based on performance and service needs.
