We are seeking an experienced and motivated Band 6 Community Psychiatric Nurse (CPN) to join our Community Mental Health Team based in Hinckley. This role offers an opportunity to provide vital care coordination to individuals experiencing mental health challenges in a supportive and collaborative environment.
Role Overview- Position: Band 6 CPN
- Responsibilities:
- Care coordination for a caseload of approximately 30 service users.
- Develop and implement person-centered care plans.
- Conduct comprehensive mental health assessments and risk management.
- Work collaboratively with multidisciplinary teams, service users, carers, and other stakeholders.
- Hours: Full-time, Monday to Friday, 9:00 AM to 5:00 PM.
- Part-time hours may be considered for suitable candidates.
- Option for one day of remote working per week, subject to agreement.
- Qualifications: Registered Mental Health Nurse (RMN) with current NMC registration.
- Experience: Demonstrated experience in care coordination within a Community Mental Health Team.
- Skills:
- Excellent organizational and communication skills.
- Ability to manage a busy and varied caseload effectively.
- Transport: Must hold a valid UK driving license and have access to a vehicle for community visits.
- Competitive hourly rate of £26 per hour.
