Are you a compassionate and experienced Band 6 Community Psychiatric Nurse (CPN) ready to make a difference in the lives of children and young people? We are looking for a dedicated professional to join our CAMHS team in Nuneaton, providing essential care coordination for young individuals with mental health needs.
Role Overview- Position: Band 6 CPN
- Responsibilities:
- Care coordination for a caseload of approximately 30 children and adolescents.
- Develop and implement individualized care plans in collaboration with service users, families, and multidisciplinary teams.
- Conduct mental health assessments and risk management plans.
- Provide high-quality support and interventions to young people and their families.
- Hours: Full-time, Monday to Friday, 9:00 AM to 5:00 PM.
- Part-time hours may be considered for the right candidate.
- Option for one day of remote working per week, subject to agreement.
- Qualifications: Registered Mental Health Nurse (RMN) with valid NMC registration.
- Experience: Proven experience in care coordination, preferably within CAMHS or a similar setting.
- Skills:
- Excellent communication and engagement skills with young people and families.
- Strong organizational abilities to manage a busy caseload.
- Transport: Must hold a valid UK driving license and have access to a car for community visits.
- Competitive hourly rate of £26 per hour.
