We are looking for a dedicated professional to join our CAMHS team in Nuneaton, providing essential care coordination for young individuals with mental health needs.

Role Overview
  • Position: Band 6 CPN
  • Responsibilities:
    • Care coordination for a caseload of approximately 30 children and adolescents.
    • Develop and implement individualized care plans in collaboration with service users, families, and multidisciplinary teams.
    • Conduct mental health assessments and risk management plans.
    • Provide high-quality support and interventions to young people and their families.
  • Hours: Full-time, Monday to Friday, 9:00 AM to 5:00 PM.
    • Part-time hours may be considered for the right candidate.
    • Option for one day of remote working per week, subject to agreement.
Essential Requirements
  • Qualifications: Registered Mental Health Nurse (RMN) with valid NMC registration.
  • Experience: Proven experience in care coordination, preferably within CAMHS or a similar setting.
  • Skills:
    • Excellent communication and engagement skills with young people and families.
    • Strong organizational abilities to manage a busy caseload.
  • Transport: Must hold a valid UK driving license and have access to a car for community visits.
