The Community Nurse Case Manager will:
- Conduct initial assessments in the community.
- Train and assess worker competencies, and allocate support workers to patients.
- Collaborate with the wider multidisciplinary team (MDT).
- Attend meetings and make referrals as needed.
- Hold team meetings and manage a team of non-clinical case managers.

To be successful as a Community Nurse Case Manager, candidates should:
- Be an RGN qualified nurse with strong clinical skills in complex care, including PEG feeds, ventilators, and tracheostomy management.
- Have experience with tracheostomy and ventilator care.
- Be a car driver, as the role involves community-based travel.
- Be flexible, working mainly Monday to Friday, 9am to 5pm, with some flexibility required.
