Job Purpose:
We are seeking a dedicated Housing Solutions Officer to join our client's team in Kingston, focusing on hospital discharge support. This role involves proactive engagement with individuals facing homelessness or housing insecurity upon hospital discharge, aiming to prevent or alleviate homelessness. You'll work collaboratively to help these individuals retain their current accommodation or secure alternative housing.

Key Responsibilities:

  • Provide advice and practical support to clients to prevent or relieve homelessness.
  • Manage a caseload, delivering tailored housing solutions in line with legal requirements.
  • Collaborate with hospitals, social services, and various agencies to facilitate smooth discharge planning.
  • Conduct home visits, attend delayed transfer meetings, and ensure compliance with safeguarding protocols.
  • Maintain accurate case records, handle inquiries, and meet performance targets.

About You:

To excel in this role, you should have:

  • Knowledge of the Housing Act 1996, Homelessness Reduction Act 2017, and relevant housing legislation.
  • Strong interpersonal and communication skills, with the ability to handle sensitive cases.
  • Experience with homelessness, housing services, and partnership working.
  • A proactive, solution-focused approach and the ability to work independently.
