- Conduct comprehensive assessments of individuals’ social care needs.
- Develop and implement tailored care plans that promote independence and well-being.
- Offer advice, guidance, and practical support to individuals and their families/carers.
- Work with healthcare professionals, community agencies, and others to deliver integrated care.
- Safeguard vulnerable adults by identifying and responding to signs of abuse, neglect, or exploitation.
- Advocate for clients' rights within the social care system.
- Maintain accurate records in line with regulatory and organisational standards.
- Participate in multidisciplinary team meetings, case conferences, and training sessions.
- Degree in Social Work or an equivalent qualification recognised by Social Work England.
- Registered with SWE as a qualified Social Worker.
- Strong knowledge of UK adult social care legislation and procedures.
- Excellent communication and problem-solving skills.
- Ability to work both independently and collaboratively in a dynamic environment.
- Empathy, resilience, and a commitment to promoting social justice.
- Proficient in IT systems used for case management and record-keeping.
