We are looking for an experienced Hospital Social Worker to join a dynamic and supportive team. If you have a passion for working with adults in complex cases and a background in hospital social work, this could be your next opportunity!The Role:You will provide critical support to unqualified staff, conduct Care Act assessments, and work on a caseload of approximately 10-12 complex cases per hospital ward. You’ll play a key role in attending ward MDTs (multidisciplinary team meetings), collaborating with other professionals, participating in best interest meetings, and discharge planning.Key Responsibilities:Caseload Management:Handle a varied caseload of complex hospital-based cases, ensuring all assessments and care plans meet the needs of the individual.Multidisciplinary Team Collaboration:Work closely with healthcare professionals in the hospital, attending MDTs and supporting discharge planning.Care Act Assessments:Carry out thorough Care Act assessments to ensure the best outcomes for service users.Support & Mentoring:Assist and guide unqualified staff and participate in team meetings to support continuous development.What We’re Looking For:Experience in Adult Social Care.Hospital social work experience is highly desirable.Driving Licence:A valid UK driving licence and daily access to a car are essential.Team Player:Ability to work well in a friendly and supportive team environment with 22 staff members, including senior social workers and managers.Strong Communication Skills:Confident in engaging with professionals, service users, and families.Interview Process:
  • Mixed competency and strengths-based interview.
