As a SEND Case Worker, you'll play a pivotal role in developing and maintaining Education Health and Care Plans (EHCPs) from initial application through to annual reviews. This is a highly rewarding position where you'll work in a person-centred, relational approach with families of children with SEND, ensuring they receive tailored, holistic support across education, health, and care.You'll be the vital link between families, schools, and partner agencies, driving a collaborative approach that ensures children and young people are educated and well-supported. Your expertise will help shape high-quality plans and interventions that truly make a difference, with a focus on reflective practice and continuous improvement.

Key Responsibilities:* Develop and maintain EHCPs, ensuring timely and high-quality assessments and plans.* Work closely with families, schools, and other key partners to create and monitor individual support plans.* Support the delivery of the local offer and engage with educational settings to drive improvements.* Manage a caseload, acting as the main point of contact for families and agencies.* Collaborate with a team around the child, responding to changing needs and circumstances.* Ensure the effective and equitable use of resources, contributing to the sustainability of services.
