Your new companyHays business support are recruiting for a Document Summariser on a part-time basis, 20-24 hours per week, based in Leicester City Centre. This is a temporary position, with the potential to be permanent. Your new roleThe Summariser is responsible for accurately summarising patient medical records and coding clinical information to support the practice's patient care and administrative processes. This role ensures that patient records are up-to-date, accurate, and coded according to NHS standards.Responsibilities include:
- Review and summarise patient records using clinical coding, including historical notes, discharge summaries, and correspondence.-Identify and record relevant medical history, diagnoses, and significant treatments on SystmOne
- Accurately apply Read Codes clinical data to ensure that diagnoses and treatments are recorded consistently.
- Collaborate with Practice Manager to clarify coding information and ensure accurate representation of patient data.
- Regularly review patient records to ensure completeness, accuracy, and adherence to confidentiality standards.
- Conduct routine audits of coded data to identify and correct inaccuracies, maintaining the highest standard of data quality.
- Experience with medical terminology and clinical coding (Read Codes etc).
- Familiarity with NHS data standards, clinical read coding, SystmOne and experience in a medical or healthcare setting.
- Strong attention to detail, organisational skills, and the ability to work independently.
- Process Subject Access Requests in compliance with relevant data protection legislation, ensuring timely and accurate responses to patients.
- Gather and compile requested information from patient records, summarising the data appropriately while maintaining confidentiality and data integrity.
