- Work collaboratively with individuals with physical disabilities, their families, and other professionals to develop and implement person-centered care plans.
- Conduct thorough assessments to identify needs and support individuals in achieving their goals.
- Advocate for individuals' rights and access to appropriate services and support.
- Promote independence and social inclusion within the community.
- Contribute to a supportive and collaborative team environment dedicated to improving the lives of our clients.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a Social Worker.
- Experience working with individuals with physical disabilities and their families is preferred.
- Excellent communication, interpersonal, and case management skills.
- A driving license and access to a car is desirable, but not essential.
- Strong commitment to person-centered care and ethical practice.
