We are seeking a skilled and dedicated Band 6 Physiotherapist to join a dynamic healthcare team in one of the Trusts in the Midlands. The Trust plays a crucial role in delivering a wide range of health services to local communities, focusing on providing care closer to home, reducing hospital admissions, and supporting patients in their communities.

Role Responsibilities:

  • You will play a key role in delivering patient-centered rehabilitation and have opportunities to work with patients at various stages of their stroke recovery.
  • Provide the specialist physiotherapy element in the Stroke Service, including clinical specialist support, professional advice, and practical guidance across Acute and Community stroke pathways.
  • Act as case manager to a designated caseload, ensuring all patients can access all elements of their care plan and that individual care plans are delivered and progressed by the team.
  • Manage a clinical caseload to a high professional standard, providing an effective evidence-based clinical service for patients as an autonomous practitioner. This includes advanced and detailed assessments for patients with diverse and complex conditions, providing a diagnosis, and developing and delivering an individualized treatment program as part of a specialist intervention.
  • Ensure a high standard of care for all patients through effective communication with patients, carers, other team members, and wider system partners.

Skills:

  • Strong clinical skills in assessing and treating musculoskeletal, neurological, or respiratory conditions.
  • Ability to create personalized and effective rehabilitation plans.
  • Excellent communication and collaboration skills within a team environment.
  • Experience in community-based or acute care settings is desirable.
