Liverpool City Council are looking to recruit a Outreach family support worker to undertake early help assessments with families, create support plans and chair team around the family meetings to review the progress and identify any additional areas of support. Day to Day duties:

  • Taking on 10 families in need of help
  • Completing EHASH referrals and meeting with families to discuss their concerns.
  • Working alongside social workers, Police and effective multi-agency teams and health professional's using relational and strengths-based practices, to deliver the right support to children young people and families.
  • Completing 'Door knocks' visiting family homes and schools checking on truancy referrals.
