Qualified Children’s Social Worker, Hospital Discharge Team Pay rate - £35 per hourVitalis are recruiting for an experienced Social Worker to work in a Hospital Team in St Helens.Experienced social worker required to work in the Integrated Discharge Team (Contact Cares) based in Whiston Hospital. This will entail assessing patients and other tasks associated with facilitating hospital discharge, to support patients with the support they require to return home or to be discharged to another setting. The role is very fast paced and high pressured and experience in this area would be preferred. This will be a full-time post (37 hours per week) between the hours of 8.45am and 5.15pm.
