Conwy Council is seeking a dedicated and empathetic Social Worker to join our Older People & Hospital Team. Key Responsibilities:
- Conduct assessments, develop care plans, and provide interventions for older adults transitioning from hospital care to community living.
- Work with multi-disciplinary teams including healthcare professionals, discharge coordinators, and community services to ensure smooth and safe discharges.
- Provide ongoing support to older adults and their families, promoting independence and well-being.
- Advocate for older individuals, ensuring their needs are met through appropriate care and services.

Requirements:
- A recognised Social Work qualification.
- Registration with Social Care Wales.
- Experience in older people's services or hospital discharge planning.
