Shift Times: 8am-4pm or 9am-5pm, Weekdays

We are seeking an experienced Locum Pharmacist to join our home service team, offering a blend of on-site and remote work. This role is ideal for a pharmacist with community and adult care experience, who thrives in a dynamic environment and enjoys flexibility.

Key Responsibilities:

  • Provide expert pharmaceutical care and services within the community and adult care settings.
  • Participate in patient consultations and manage medication therapy to ensure optimal outcomes.
  • Support both on-site and remote pharmacy services, ensuring timely and accurate medication delivery.
  • Liaise with healthcare teams to coordinate care for patients across various specialties.
  • to deliver a high-quality Medicines Optimisation Service to improve medicines-related outcomes for high-risk patients within the frailty Hospital @ Home caseload and ensure the safe, cost-effective, and evidence-based use of medicines
  • Work closely with the other Hospital @ Home pharmacists across the trust to support each other and ensure standards and goals are transcended across services.

Requirements:

  • Registered Pharmacist with experience in community and adult care.
  • Flexibility to work both on-site and remotely (hybrid role, but majority on-site).
  • Car required for patient visits and service delivery.
  • Strong communication and organizational skills.
  • Ability to start immediately and commit to a 4-week assignment.
