What You’ll Be Doing:Join our team as a CHC Assessor, where you’ll play a key role in ensuring individuals receive the right level of care. In this role, you will:
• Conduct comprehensive Continuing Healthcare (CHC) assessments• Work closely with multidisciplinary teams to review care needs• Ensure compliance with National Framework guidelines• Provide detailed reports and recommendations for funding decisions• Engage with patients, families, and healthcare professionals
Requirements & Qualifications:To be successful in this role, you will need:
• A valid NMC/HCPC registration• Previous experience in CHC assessments• Strong knowledge of the National Framework for CHC• Excellent communication and decision-making skills• The ability to work efficiently in an onsite setting
Working Hours & Contract Duration:This is an ongoing locum opportunity with onsite working in North London. Flexible contract lengths are available, with a strong possibility of extension based on performance and service needs.
