Job Description:

We are seeking a dedicated and experienced Band 7 Clinical Pharmacist to join a dynamic team in the Specialist Medicine Wards of an NHS Hospital in East London. This is an exciting opportunity for a motivated professional to provide high-quality pharmaceutical care and make a meaningful impact on patient outcomes.

Key Responsibilities:

  • Conduct medication reviews and ensure the safe and effective use of medicines within the specialist medicine wards.
  • Provide clinical pharmacy input to multidisciplinary teams, participating in ward rounds and patient consultations.
  • Develop and implement medication management plans tailored to individual patient needs.
  • Educate and counsel patients and healthcare staff on medication therapies, potential side effects, and best practices.
  • Maintain accurate medication records and ensure compliance with hospital policies and regulatory requirements.
  • Participate in audits, service evaluations, and quality improvement initiatives.
  • Mentor and support junior pharmacists and pharmacy technicians.

Requirements:

  • Registered pharmacist with the General Pharmaceutical Council (GPhC).
  • Relevant experience in a clinical pharmacy role, ideally within specialist medicine.
  • Strong clinical knowledge and the ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
  • Commitment to continuous professional development.
