The Nurse Case Manager is responsible for ensuring client's receive high standards of care delivered by carer safety within their caseload.Your job:Working as a Nurse Case Manager is a timely opportunity to join a pool of dedicated nurses who are responsible for ensuring the delivery of high-quality person-centred care across South London. All of our care and attention is delivered in clients' homes by skilled and experienced Healthcare Assistants.
