About the Role

We are seeking an experienced Case Management Officer to join our busy Front Door Service. The ideal candidate will have previous experience in Front Door or Hospital Discharge services, as well as a strong understanding of the high demands within Adult Social Care (ASC). You will play a key role in supporting individuals, their families, and carers to make informed choices, live independently, and achieve their well-being goals.

Key Responsibilities
  • Act as the main point of contact for individuals requiring Adult Social Care support.
  • Complete assessments and reassessments to determine care and support needs.
  • Work collaboratively with health, housing, and community services to ensure effective service delivery.
  • Support individuals in accessing appropriate services, promoting independence through a strength-based approach.
  • Provide high-quality information, advice, and signposting to individuals and carers.
  • Manage casework, including safeguarding concerns and crisis interventions, in partnership with other professionals.
  • Ensure accurate recording of case details in line with legislation and best practices.
  • Facilitate and contribute to multidisciplinary meetings to coordinate support plans.
  • Promote self-directed support and direct payments to service users and carers.
  • Work effectively under pressure, balancing urgent cases within a varied caseload.
About You

We are looking for someone with:

  • Experience in a frontline Adult Social Care environment, preferably in Front Door or Hospital Discharge services.
  • A strong understanding of The Care Act 2014, eligibility criteria, and safeguarding responsibilities.
  • Knowledge of assessment procedures, personalised care planning, and strength-based approaches.
  • Excellent organisational skills and the ability to manage a demanding caseload.
  • Strong written and verbal communication skills, with the ability to engage with service users, carers, and professionals.
  • IT proficiency, including experience using case management systems.
  • A proactive and flexible approach to working in a fast-paced environment.
  • The ability to work collaboratively with different teams and external agencies.
