Job Description:

We are seeking a dedicated and compassionate Homeless Housing Coordinator to join our team. This vital role will oversee the coordination and implementation of housing programs for individuals and families experiencing homelessness. The Housing Coordinator will work closely with local agencies, service providers, and clients to assess needs, facilitate housing placements, and ensure access to necessary services for successful housing retention.

Key Responsibilities:

  • Assess the housing needs of individuals and families experiencing homelessness.
  • Develop and implement personalized housing plans, working closely with clients to ensure a smooth transition into stable housing.
  • Coordinate housing placements, working with landlords, property managers, and other housing partners.
  • Provide case management support to ensure clients are connected to necessary services, including mental health, substance abuse, job training, and more.
  • Monitor clients' housing stability and provide ongoing support to prevent homelessness recurrence.
  • Collaborate with community organizations, government agencies, and housing providers to identify resources and advocate for policies supporting homeless individuals and families.
  • Maintain accurate records and reports regarding client progress, housing placements, and service coordination.
  • Organize and facilitate outreach efforts to engage individuals in need of housing services.
  • Stay informed on best practices, trends, and policies in homelessness services.
