The role:

Families First Coach's are case holding and work relationally and restoratively with families, providing intensive support to bring about change and ensure positive outcomes for children and families on the cusp of statutory social work processes.

Families First Coach's support families to anticipate future challenges and skills that they have learnt and can utilise around challenges of family life (relapse prevention). Encouragement is given to apply these new skills in different contexts, inside and outside of the home so that the family can be empowered to feel confident about maintaining the progress they have made.

The purpose of the Short Breaks Practitioner in the Children and Young People's Disability Service is to carry out high quality and strengths-based short breaks assessments and reviews of social care packages of vulnerable children and young people with severe to profound disabilities and/or complex health needs. The Short Breaks Practitioner works with the child and their family to ensure positive outcomes and enable the child to reach their full potential. The post holder will facilitate and capture the views of the child / young person they work with to inform future service development and any changes in local procedures.
