Job Overview:We are seeking an experienced and self-motivated Band 6 Community Occupational Therapist to join our team. This role involves providing early discharge support, community rehabilitation, equipment provision, and care package reviews to enhance patients' independence and quality of life. The ideal candidate will have a broad clinical background and experience working in diverse community settings.
Key Responsibilities:
- Conduct comprehensive assessments and provide tailored occupational therapy interventions in the community.
- Support early discharge by facilitating rehabilitation plans and coordinating with hospital teams.
- Provide and review specialist equipment to promote independence and safety at home.
- Review and update care packages in collaboration with patients, families, and carers.
- Develop individualized care plans to address patients' physical, functional, and social needs.
- Work closely with multidisciplinary teams to ensure holistic patient care.
- Deliver education and advice to patients and families on managing long-term conditions.
Requirements:
- Degree or equivalent in Occupational Therapy and current HCPC registration.
- Minimum 6 months of NHS experience, including community occupational therapy.
- Proven experience in early discharge planning, community rehabilitation, and equipment provision.
- Strong communication, organizational, and time-management skills.
- Ability to work independently and collaborate effectively within multidisciplinary teams.
- Full UK driving license and access to a vehicle is desirable.
